SSMHA Player Evaluations (Try Out) Registration Form 2024.25
Please complete the online Player Evaluation form to attend the South Simcoe Minor Hockey Association Player Evaluations. Your confirmation number is your player number which can be found at the bottom of your email confirmation - it will say confirmation page southsimcoeminorhockey.ca/submissions/player number. This number will also be emailed to you closer to the skates. The SSMHA Passport fee is $125. Please e-transfer [email protected] payment upon registration. Failure to pay will result in the player being removed from the Player Evaluation Skates (Try Outs). Please remember to include the player name, division and level (AA, A, BB) in the e-transfer. Team Selection

Residentially qualified players (Bradford, or Innisfil) can be released to the division below after the first Player Evaluation. Players will then report to the next division below at no extra cost to continue Representative Player Evaluations. Players can be evaluated at any level they feel best fits their needs. BB will be a non-hitting category for U14, U15, U16, and U18 for those wishing to play without contact. U10, U11, U12, and U13 representative players not offered a spot on a AA or A team will be eligible to be evaluated for a team that will play at a category below A that is yet to be determined. The category will be determined prior to the start of the season.

NRP - Non Resident Players

Non Resident Players cannot participate without an approved NRP card. NRP players must qualify as per OMHA regulations, and must adhere to all OMHA regulation as per the OMHA Manual of Operations. NRP players will receive a minimum of 2 evaluation skates. If released they would go back to their home centers

Passport Fee

The SSMHA Passport fee is $125. Please e-transfer [email protected] payment upon registration. Failure to pay will result in the player being removed from the Player Evaluation Skates (Try Outs). By submitting this registration form you have placed your child on the player evaluation list for the selected team.

Payment is due upon registration.  Failure to provide payment will result in removal from the list.  

NRPs MUST present their card at the sign in table.

I certify the information submitted on this registration form to be true and that I have become subject to the rules, regulations and decisions of Hockey Canada/OMHA and South Simcoe Minor Hockey,  and I agree to abide by such rules, regulations and decisions of Hockey Canada/OMHA and South Simcoe Minor Hockey.  I understand that the South Simcoe Minor Hockey Association will not be held responsible for any injuries while participating in league activities.

By registering for tryouts with South Simcoe Minor Hockey representative teams, we acknowledge that we have read and agree to comply with the Association's Player Evaluation Procedure and Guidelines.  We understand that tryouts are for competitive teams and that players selections are based on many factors and are ultimately at the exclusive discretion of the Association. We agree to respect any decisions made as a result of the player evaluation process and waive any right of complaint or appeal to the OMHA or otherwise.
